The Quick Read: No dentist-only HELOC tier exists, so the right program is the one that fits your collateral, your credit profile and the way your practice income is documented. Lenders underwrite you personally, not the practice. Equity in the property, a clean credit file and income shown after deductions decide the outcome.
- Occupancy sets the ceiling: investment properties top out at 70% CLTV, while primary residences can reach higher tiers.
- Practice owners often show low taxable income because of depreciation and write-offs, so the documentation path matters as much as the credit score.
- The network’s lines are drawn mostly at closing, so this is not a standby backup line.
- Properties held in an LLC cannot carry these lines, which catches many practice owners.
- A DSCR loan is a separate lane for rentals, built on the property’s rent rather than your practice income.
What “Best” Means for a Practice Owner
“Best” is a fit test, not a single winner. A HELOC is a home equity line of credit, which is a revolving loan secured by the equity in a property. Equity is the property’s value minus what you owe on it.
How large a line the equity supports.
An equity line is sized by combined loan-to-value, occupancy, and credit — not by rental coverage. Switch the occupancy or the credit band and the ceiling moves with it.
Investment-property lines require a 700 minimum credit score; second-home lines start at a 640 minimum and primary-residence lines at 600, and the combined-LTV ceiling steps down as the credit band drops on primary-residence and second-home lines and holds on investment-property lines; the line cap steps down on primary-residence lines and holds at every tier on investment-property and second-home lines.
A debt-to-income ratio above 45% requires 680+ credit. Profiles under 640 are limited to single-family homes. At least 75% of the approved line is drawn at closing. Ceilings, floors, and caps update from Lendmire’s centralized guideline source.
Line estimate
Illustrative estimate only — not a quote, Loan Estimate, approval, or commitment to lend. The rate is an editable assumption; equity-line pricing is variable through both the draw and repayment periods and never converts to fixed. Occupancy and credit drive the ceiling together: an investment property tops out at 70% combined LTV (minimum credit 700, line cap $500,000); a second home tops out at 90% combined LTV (minimum credit 640, line cap $500,000), with the ceiling stepping down as the credit band drops (the cap holds at every tier); a primary residence tops out at 90% combined LTV (minimum credit 600), and its $750,000 maximum line is available only at 75% combined LTV or below with a 700-or-better credit profile (720 on the longer-runway program) and a full appraisal. Lines above $500,000 require a full appraisal. Credit, debt-to-income, property type, and full underwriting review all affect the final line.
Lendmire is a mortgage broker, not a lender. It places files with select wholesale lenders and matches the scenario to the program. The best program for your situation depends on the property you borrow against, your credit, how much leverage you need and how you document income.
Dentists get no special treatment in this product. A lender sees a self-employed borrower with equity and a credit file. The profession label does not change the terms, but the way your income is structured does.
How Underwriting Treats a Dentist, Step by Step
Here is the sequence a file follows.
1. Pick the collateral. The line sits on a property you own in your own name. That is usually your primary residence, a second home or an existing rental.
2. Size the line. The lender applies a maximum combined loan-to-value (CLTV) to the property’s value. CLTV adds your first mortgage and the new line together, then divides by value. The line is what fits under the cap.
3. Check credit. The network’s program floor is a 600 score, but the higher tiers need much more. The score model keys to the primary wage earner, and the report can be no more than 90 days old at closing.
4. Document income. Full documentation uses your returns, which show income after deductions. A bank-statement lane exists for owners whose returns understate what the practice actually produces. Lendmire’s piece Best Bank Statement HELOC Program as a Dentist or Practice Owner covers that path in detail.
5. Test the debt-to-income ratio. DTI compares your monthly debts to your income. The network allows up to 50%, and 45% for credit profiles from 600 to 679. Anything above 45% requires a 680 score. The test runs on the interest-only payment calculated on the maximum draw, not on what you plan to borrow.
6. Value the property. Lines at or below $500,000 ordinarily use an automated valuation instead of a traditional appraisal. A higher CLTV may require a second valuation. Every line above $500,000 requires a full appraisal, and you can request one in any case.
Practice owners often struggle at step 4. Equipment write-offs, depreciation and owner-compensation structure can make a profitable practice look thin on paper. Underwriters use net income, not gross collections.
What Are the Program Limits by Occupancy?
The ceiling depends on the occupancy of the property, and your credit tier moves it further. Here is the headline view for lines at or below $500,000.
| Collateral | Max CLTV | Credit needed |
|---|---|---|
| Primary residence | 90% | 720+ |
| Primary residence | 85% | 660 to 700+ |
| Second home | 90% | 720+ |
| Investment property | 70% | 700+ |
Primary residences also reach 75% CLTV on lines up to $750,000, which is the program maximum. A line above $500,000 is for primary residences only. It requires a 700 or better credit profile (720 on the longer-runway structure described below) and caps at 75% CLTV.
Investment property lines run to $500,000 at a 70% CLTV ceiling, with a 700 minimum score. Lower credit tiers cut the primary-residence numbers sharply. At a 620 score, for instance, the ceiling falls to 70% CLTV on a line up to $400,000.
Here is the math in percentages. Say your first mortgage equals 60% of a home’s value, and your credit tier caps CLTV at 85%. The line can use up to 25% of the value. On an investment property with a first mortgage at 55% of value, a 70% cap leaves 15%.
These figures are guidance from select wholesale lenders. Every file gets individual review, and nothing here is a commitment to lend.
Two Draw Structures, and Why Both Matter
The network offers two wholesale structures. Both are standalone lines, in first or second lien position.
The shorter-draw program has a 3-year interest-only draw followed by a 17-year repayment period. Investment lines use the longer-runway structure only. Tennessee shortens both structures.
Two rules shape how you plan. First, at least 75% of the line is drawn at closing on both programs. That makes this a funding tool for a known need, not a safety net you leave untouched. Second, the pricing floats through the draw and the repayment period and never converts to fixed. Plan your cash flow for payments that move.
After closing, the minimum subsequent draw is $1,000 on the longer-runway program, except in Texas, where it is $4,000. The shorter-draw program does not state subsequent-draw minimums.
The CFPB describes the standard HELOC pattern the same way: a draw period for borrowing, then a repayment period where the balance comes due on a schedule. Know the date when your payment steps up before you spend a dollar.
Which Program Fits Which Practice Owner?
The established owner with a primary residence. This is the strongest profile. Equity is large, credit is usually high and the primary-residence tiers have the most room. The open question is income documentation, since heavy deductions can squeeze the DTI test.
The new owner or recent buyer. A short ownership history is a common hurdle for self-employed borrowers in general. A practice acquisition loan also carries its own debt service, which cuts into your DTI cushion. Expect the file to lean on your credit and equity.
The associate or production-based dentist. Income is lumpy, and the structure of your pay affects what counts. Documented income under the applicable program, subject to lender guidelines, is the test. Lendmire’s article How to Qualify for a DSCR HELOC as a Dentist or Practice Owner walks through the rental-property angle.
The owner who holds property through an LLC. Fee simple or leasehold title must sit with you personally, or in an inter vivos revocable living trust. LLCs, corporations, partnerships and irrevocable, blind or land trusts cannot hold title. A property already deeded to an LLC needs a vesting change or a DSCR cash-out refinance instead. This is the sharpest structural difference from a DSCR loan, and many practice owners hit it.
Where the General Rule Breaks
Several edge cases change the answer.
- Commingled accounts. On bank-statement files, personal and rental money in the practice account forces the lender to exclude deposits one by one. Keep the accounts separate before you apply.
- Renting out the house. The CFPB booklet on HELOCs warns that your line’s terms may prohibit renting out the home, and that the lender may reduce the line if the home’s value falls. Read your line agreement before you convert a residence to a rental.
- Low-credit profiles. Sub-640 profiles are limited to single-family residences with a clean 12-month housing history under the longer-runway program. Because second homes floor at 640 and investment at 700, that restriction reaches primary residences only.
- Past credit events. A bankruptcy needs 4 years of seasoning from discharge or dismissal on both programs. For foreclosure-family history, the programs split on primary residences and second homes. One seasons a foreclosure at 7 years and a deed-in-lieu, pre-foreclosure or short sale at 4. The other declines the history entirely.
- Portfolio size. A borrower is limited to three lines. Combined exposure caps at $2,000,000 on the shorter-draw program and $750,000 on the longer-runway program. A borrower with more than 15 financed properties is not eligible.
- Property type. Single-family, 2-4 units (640 minimum on the longer-runway program), PUD, townhome and condominium, including non-warrantable condos, are eligible. Modular factory-built homes are eligible on the longer-runway program only. Manufactured homes, co-ops, condotels, log homes, commercial, mixed-use and agricultural zoning are not eligible on either program.
- Geography. Lendmire’s HELOC availability covers its 16 full-service states: AL, CA, CO, FL, GA, IN, MI, MT, NM, NC, OH, PA, TN, TX, VA and WA. That is narrower than its 41-market DSCR footprint.
The general market is tighter than any one program suggests. NerdWallet notes that a HELOC on an investment property is possible, but the pool of lenders is smaller, the borrowing limit is lower than against a primary home and the requirements are strict. That matches what brokers see: this is why matching the file to the right program matters.
When a DSCR Loan Beats a HELOC
Practice income is the weak point for many dentist files. A DSCR loan sidesteps it for the rental itself. A DSCR loan is a business-purpose investor loan that qualifies primarily on property-level rental income covering the payment, subject to lender guidelines. Your practice income does not set the outcome. The complete DSCR loans guide covers the full mechanics.
Typical DSCR figures from select wholesale lenders look like this:
- Purchase leverage mostly lands at 75% to 80% LTV, and select programs reach 85% LTV at roughly a 700+ score.
- Cash-out refinance on a standard rental tops out around 75% LTV, with about 6 months of seasoning as the common expectation.
- Coverage of 1.00 is where select programs start. Stronger ratios open better pricing and leverage. A separate select-lender path takes coverage below 1.00, with leverage and terms adjusted.
- Credit floors run from 620 in parts of the network to roughly 660 for most programs, and 700+ unlocks the strongest leverage tiers.
A DSCR ratio compares rent to the full monthly payment. Clearing 1.00 does not mean the property produces positive cash flow. Repairs, vacancy, management and capex sit outside the calculation.
Here is the practical split. A HELOC keeps your first mortgage in place and adds a second payment that floats. A cash-out refinance replaces the first mortgage with a new one. If you already hold a low-cost first mortgage on a rental, a line may make sense. If the property is in an LLC, or you want the rental’s income to carry the loan, the DSCR route is usually the cleaner fit.
What Is the Real Risk?
The home secures the debt. The CFPB states that if you fall behind, you could lose your home. A dentist who borrows against a personal residence to fund a rental or a practice need is putting the home at risk for that purpose.
The floating payment adds a second stress point. Run your numbers at the maximum draw, because the network qualifies you that way. Then ask whether the practice and the rental can both carry it through a slow quarter.
Tax treatment can depend on how the funds are used and how the property is held; borrowers should keep clear records and speak with a qualified tax professional before relying on any deduction.
Key Terms Defined
CLTV: Combined loan-to-value is your first mortgage plus the new line, divided by the property’s value.
Draw period: The stretch of time when you can borrow against the line, with interest-only payments in this network.
Repayment period: The later stretch when you pay down principal and interest on a fixed schedule.
DTI: Debt-to-income compares your monthly debt payments to your monthly income.
DSCR: Debt service coverage ratio compares a rental’s income to its monthly payment (principal, interest, taxes, insurance and any HOA dues).
Vesting: How the property’s title is held, whether in your name, a trust or an entity.
Frequently Asked Questions
Can a dentist get a HELOC on a rental property?
Yes, under a narrower set of rules. The network’s investment lines top out at 70% CLTV, require a 700 minimum score and cap at $500,000. The property must be titled in your own name or a revocable living trust, so a rental held in an LLC does not qualify without a vesting change.
Do lenders count my gross collections as income?
No. Underwriters work from documented income after deductions, not gross revenue. Bank-statement programs exist because deduction-heavy returns can understate cash flow. The applicable program and lender guidelines decide how deposits are counted.
How much can I borrow against my primary residence?
It depends on your credit tier and existing mortgage. At a 720 score, the program reaches 90% CLTV on lines up to $500,000, or 75% CLTV on lines up to $750,000. Lower tiers cut the ceiling, and your first mortgage uses part of the room.
Can I leave the line mostly undrawn as a backstop?
Not on these programs. At least 75% of the line is drawn at closing on both, and the pricing floats through the draw and repayment periods. A line you never intend to use is a better fit for a different product.
Should I use a HELOC or a DSCR cash-out refinance?
A line fits a staged or uncertain need while you keep the first mortgage. A cash-out fits a one-time lump sum from a rental you already own. Cash-out files typically tie to about 6 months of seasoning, and each case goes through individual lender review.
Next Step
If you are considering a home equity line and want to see how the numbers work, Lendmire can help you compare HELOC options based on the property, the equity available, credit profile, combined leverage and your goals. Practice owners usually get the cleanest result by choosing the collateral first, then matching the documentation path to it. Equity matters, but so does how the next rental’s own income carries its payment.
