Banks That Offer a Short-Term Rental Cash-Out Refinance to Cover a Medical Bill

Banks That Offer a Short-Term Rental Cash-Out Refinance to Cover a Medical Bill

The Quick Read: A DSCR cash-out refinance on a short-term rental is built for business purposes, and a personal medical bill is not a business use of the money. The programs in Lendmire’s wholesale network generally prohibit using cash-out proceeds to pay personal debts, so the loan is rarely the right tool for this job.

  • Short-term rental cash-out loans typically top out at 70% LTV on a cash-out refinance, subject to lender guidelines.
  • Expect a 640+ credit score and about 12 months of hosting history on most files.
  • Select programs start at a coverage ratio of 1.00, and a separate select-lender path takes coverage below that, with leverage and terms adjusted.
  • Large banks that do a personal-purpose cash-out or equity product work from a different, income-based file.
  • A mortgage broker shops multiple wholesale lenders and matches the scenario to the program. That is where Lendmire fits.

Medical bills arrive on their own schedule, and a rental with equity sitting in it looks like an obvious answer. The instinct is understandable. The loan structure is the problem, not the property.

Key Terms Defined

Cash-out refinance: a new loan that pays off the old one and hands you the leftover equity as cash.

DSCR (debt service coverage ratio): the property’s monthly rent divided by its monthly payment of principal, interest, taxes, insurance, and association dues (together called PITIA).

Business-purpose loan: a loan made for an investment or commercial use, not for personal or household spending.

Seasoning: the waiting period a lender wants between buying a property and refinancing it. About 6 months is the common expectation across most of the network.

Reserves: liquid savings a lender wants you to hold after closing, commonly around 6 months of PITIA, though the number varies by lender, leverage, and loan size.

LTV (loan-to-value): the loan balance as a percentage of the property’s appraised value.

Why Purpose Decides Everything Here

The short answer: this loan is reviewed on the property’s rental income covering the payment, but the money has to go somewhere that fits a business loan. Medical bills do not.

DSCR loans are designed for non-owner-occupied investment properties. Because they are business-purpose investor loans, they are reviewed differently from a standard owner-occupied mortgage. Underwriting looks at the property’s income, not your paycheck. The trade is that the loan has to serve the business.

Across the wholesale network, the guidelines prohibit using cash-out to pay personal debt. Typical examples are personal credit cards, personal tax liens, and personal collections. Allowed uses look like property improvements, a down payment on another investment property, or paying off investment-related debt. A medical bill is personal. It sits on the wrong side of that line.

The Doss Law guide to the business-purpose exemption uses a cash-out refinance of a single-family rental to pay personal income taxes as its example of a consumer-purpose loan, and a medical bill is the same kind of use. Per Doss Law’s business-purpose guide, those consumer-purpose loans on 1-4 unit property fall under the disclosure and ability-to-repay rules that business-purpose loans avoid.

So the problem is not that lenders are being fussy. A loan sold as business-purpose cannot honestly be used for a personal bill.

How Underwriting Treats a Short-Term Rental Cash-Out, Step by Step

Here is the sequence a file follows, with the use-of-proceeds screen in the middle.

1. Eligibility. The property must be non-owner-occupied investment real estate. You sign attestations that the loan is for business purposes and that you will not live there. Manufactured homes, log homes, and barndominiums are not offered in these programs.

2. Use of proceeds. The file states what the cash is for. Improvements, another investment down payment, and investment-related debt fit. Personal bills do not.

3. Income determination. For a short-term rental, lenders usually want about 12 months of hosting history. That means platform statements for Airbnb or Vrbo, often matched to bank deposits. Some programs may also weigh a market projection or a long-term rent figure from an appraisal rent schedule, and many use the lower supported number.

4. The coverage test. Rent is divided by PITIA. Select programs start at 1.00 on a short-term rental refinance. A stronger ratio can improve pricing and terms.

5. The proceeds cap. You get the smaller of two numbers: leverage times appraised value, or the loan the cash flow supports. On short-term rental collateral, cash-out tops out at 70% LTV, a notch below the 75% ceiling on standard rentals. A bigger appraisal does not guarantee the cash you asked for.

6. Credit and reserves. Credit is pulled and still drives pricing. Short-term rental files generally want a 640+ score, and reserves are commonly around 6 months of PITIA, varying by lender, leverage, and loan size.

7. Documents. Expect the platform history or lease, credit report, appraisal, title work, and a payoff statement.

Clearing 1.00 does not mean the property produces positive cash flow. DSCR compares rent to PITIA only. Repairs, vacancy, management, utilities, and capital expenses sit outside the calculation.

Why an LLC Does Not Change the Answer

A common assumption is that vesting the property in an LLC makes the purpose question irrelevant. It does not. LLC-titled borrowers may be eligible, subject to lender program eligibility, but the use-of-proceeds rules still apply.

Hunton’s analysis of business-purpose loans notes that loans to entities are generally exempt from the consumer rules. It also warns that “business purpose” does not mean “compliance exempt,” and that lenders face penalties for wrongly treating a loan as exempt. A lender that knows a medical bill is the real reason for the cash is not going to paper it as something else, and you should not ask it to.

The Structures and Variations That Exist

The short-term rental programs in the network share a spine. Terms vary after that.

Feature Short-term rental purchase Short-term rental cash-out
Max LTV Up to 75% Up to 70%
Min credit score 640 640
Coverage floor 1.00 1.00

Beyond leverage, structure options include the 30-year fixed as the base, with extended terms such as 40-year and interest-only periods available through select lenders. ARM structures exist for investors who want them. Loan sizes run up to $3,000,000 on standard programs, with balances below the standard-program minimum routed through select lenders in the network, and above $2,500,000 the network generally holds to 30-year fixed.

Coverage below 1.00 is a real path too. It is available through select lenders in the network, with leverage and terms adjusted. It does not change the use-of-proceeds rule. Likewise, a larger down payment on a purchase lowers the payment and can lift the ratio, but it never erases leverage caps, credit floors, reserve rules, or property eligibility.

A rate-and-term refinance is the relevant variant for some investors. It replaces the existing loan without distributing meaningful cash, so it usually carries fewer restrictions on proceeds. It will not put money in your hand for a hospital, though it can lower the payment on the property and ease monthly pressure.

Where the General Rule Bends

A few edge cases are worth knowing, along with the limits on each.

Timing of the bill. Lenders judge purpose at origination. A medical bill that already exists when you apply is a personal use of proceeds. An emergency that arises after closing is a different situation, and it depends on program guidelines. Do not plan a loan around the second scenario. Tax treatment varies; consult a qualified professional.

Owner-occupancy. If you plan to live in the property, DSCR programs generally do not apply at all. The CFPB’s Regulation Z, section 1026.3 treats credit to acquire, improve, or maintain a non-owner-occupied rental as business-purpose, and it applies different rules when the borrower will occupy the property. That reasoning covers the rental itself. It does not stretch to a personal bill paid from the proceeds.

Unpermitted or illegal short-term use. Short-term rental rules can vary by city, county, HOA, and property type, so investors should confirm local rules before relying on projected rental income. Income from a property without legal short-term use may not count at all.

Prepayment penalties. Many DSCR loans carry prepayment penalties in the early years. Refinancing out of a loan you just took, to chase cash, can get expensive.

So Where Do “Banks” Fit?

The honest answer is that the word “banks” in this search points at a different product. Large banks and depository lenders generally do not run DSCR programs on short-term rentals. Some offer a consumer cash-out or home equity product secured by an investment property. That is a personal-purpose, income-based loan.

Those loans work differently:

Factor DSCR cash-out Bank consumer product
Reviewed on Property rental income Personal income and DTI
Purpose Business only Personal use allowed
Documents Platform history, credit, appraisal Tax returns, pay records
Short-term rental income Counted under program rules Often discounted or excluded

For a medical bill, the bank-style product matches the purpose. The tradeoff is documentation and a debt-to-income calculation, and short-term rental income is often harder to count there. Which route fits depends on your income documents, your other debts, and how the property earns.

Lendmire is a mortgage broker, not a lender. It does not name or rank banks, because the right fit depends on the file. It arranges DSCR financing through select lenders in its wholesale network across 41 markets, including Washington, D.C., and it will tell you plainly when a DSCR loan is the wrong tool.

What the Decision Looks Like in Practice

Picture an investor with a paid-down Airbnb property, a seasoned hosting history, and a large hospital bill. Walk the decision in order.

First, ask what the money is for. If it is the bill, a DSCR cash-out is off the table, and the conversation moves to personal-purpose options.

Second, consider the cheaper moves before you put a lien behind medical debt. Ask the hospital about itemized review, a payment plan, and financial assistance. Securing medical debt with property raises the stakes, because missed payments put the asset at risk.

Third, if you do have a business reason to pull equity, such as renovations that may support a higher appraised value depending on comps and underwriter review, the DSCR path works. Final terms depend on lender guidelines, property type, leverage, and the borrower’s complete credit picture.

Fourth, protect the file for later. Keep platform statements, deposit records, and a written record of how proceeds were spent. The 12-month history can be the deciding document.

Finally, run the numbers. Lendmire’s calculator gives an illustrative estimate of coverage and leverage, and a broker can match the scenario to the program. Quote requests start at 828-256-2183, or through Lendmire’s quote page.

Frequently Asked Questions

Can I use a DSCR cash-out refinance on my Airbnb to pay a medical bill?

Generally no. The network’s guidelines prohibit using cash-out to pay personal debt, and a medical bill is personal. Programs are underwritten individually, so ask a broker about your exact file, but expect the purpose to be stated on the application.

Do banks offer short-term rental cash-out refinances?

Large banks mostly offer income-based consumer products, not DSCR loans. Their files use personal income documents and a debt-to-income calculation. DSCR lenders in a wholesale network qualify the loan primarily on property-level rental income covering the payment, subject to lender guidelines.

What is the maximum cash-out on a short-term rental?

Short-term rental cash-out tops out at 70% LTV on most files, compared with about 75% on standard rentals. The actual proceeds are the smaller of the leverage-based amount and what the cash flow supports. A higher appraisal does not guarantee more cash.

Does a higher DSCR let me borrow more on a short-term rental?

A stronger coverage ratio can improve pricing and terms, but it does not lift the cash-out ceiling. Leverage caps, credit floors, and reserve rules still apply. The strongest files have both enough equity and enough rental coverage.

What if my hosting history is under a year?

Most short-term rental programs want about 12 months of history. With less, some lenders may rely on a more conservative long-term rent figure, and a broker can show which programs fit.

If you are buying or refinancing a rental property and want to see how the numbers work, Lendmire can help you compare DSCR loan options based on the property income, credit profile, leverage, and investor goals.

A medical emergency is exactly when you want the cleanest, more affordable option, and for most investors that starts with the hospital’s billing office, not the rental’s equity.
