How to Finance a Hospital-Area Airbnb With a DSCR Loan

How to Finance a Hospital-Area Airbnb With a DSCR Loan

The Quick Read: A hospital-area Airbnb is financed with a DSCR loan by qualifying the property’s rental income against its full monthly housing payment, not your personal paycheck, and the outcome depends on how the lender counts that income and whether short-term use is allowed at the address. Most of the work happens before the loan application. You have to prove the income is real and the use is legal.

  • Short-term rental purchases typically reach up to 75% LTV on the strongest files, with refinances around 70%, subject to lender guidelines.
  • Expect a credit score of about 640 or higher and roughly 12 months of hosting experience on most short-term rental files.
  • Furnished stays of 30 days or more, such as travel-nurse contracts, are often read as long-term rent, not Airbnb income.
  • Clearing the coverage ratio does not mean the property makes money after repairs, vacancy and management.

Hospital-area properties sit in an odd spot. The guests are real and the demand is steady in many places. But the demand is tied to one employer cluster, and the paperwork often does not match how the income actually arrives.

What Does a DSCR Loan Actually Measure on a Short-Term Rental?

A DSCR loan measures one thing: whether the property’s income covers the property’s monthly obligation. The ratio is monthly income divided by PITIA, which is principal, interest, taxes, insurance and any association dues. The loan is reviewed primarily on property-level rental income covering the payment, subject to lender guidelines.

That is why DSCR loans suit investors who cannot or do not want to run their file through traditional personal-income documentation. Self-employed owners, entity buyers and people with several financed properties often fit here. A salaried buyer with one rental may find a conventional loan cheaper. The flip point is usually the property count or the messiness of the personal income picture.

DSCR loans are designed for non-owner-occupied investment properties. Because they are business-purpose investor loans, they are reviewed differently from a standard owner-occupied mortgage.

Across the wholesale network Lendmire works with, 1.00 is where many select programs start. A separate select-lender path goes below 1.00, with leverage and terms adjusted. Stronger ratios generally open better pricing and leverage. The complete DSCR loans guide covers the full mechanics if you want the baseline first.

Why Is a Hospital-Area Airbnb a Distinct Case?

It is distinct because the guests are not typical vacationers. Demand near a hospital usually comes from traveling clinicians, visiting medical staff, remote professionals, graduate students and people in town for medical appointments. It tracks the hospital’s staffing needs more than a tourism calendar.

That has two underwriting effects. First, many of these stays run for weeks, which pushes the file toward the long-term side of the line. Second, the comparable listings that support a projection may be few, because hospital streets are not always dense with nightly rentals.

Extended-stay hotels nearby are a useful demand signal. So is a mix of guest types: a property that depends on a single guest type is fragile.

On the market side, Staffing Industry Analysts forecasts travel nurse revenue edging up about 1% to $14.3 billion after a three-year decline. Read that as stabilized, not booming.

How Does Underwriting Treat the Income, Step by Step?

Underwriting follows a fixed sequence: classify the income, choose the evidence, order the appraisal, run the ratio, then check eligibility. Each step can change the number that goes on top of the ratio.

1. Classify the stay type. Most programs sort a file into long-term rent or short-term rent. Only the income figure changes. The payment side stays the same.

2. Choose the income evidence. A true short-term rental can be supported by trailing platform revenue, a market-data projection, or the appraiser’s rent estimate.

3. Order the appraisal. It sets the value that drives LTV. It also gives a market-rent opinion. The appraiser’s rent schedule is Form 1007 for one-unit properties and Form 1025 for two-to-four units, and DSCR files commonly use them that way.

4. Run income divided by PITIA. The lender takes the income figure it accepts and divides it by the full payment, including taxes, insurance and dues.

5. Review eligibility. Credit, leverage, reserves, property type and use are checked against the program.

Traditional personal-income documentation and employment documentation is typically not part of this file. The property’s income, the appraisal, entity documents and insurance still are. Do not read DSCR as a loan that skips paperwork.

Which Income Evidence Works for Hospital-Driven Stays?

The right evidence depends on whether the property already has booking history. A property with 12 months of hosting records is judged on actuals. A new purchase usually leans on a projection or the appraiser’s figure.

Method What it is Fits when Risk near a hospital
Platform history Trailing booking revenue Existing host, full year of records Reflects one guest mix; may include a one-time surge
Market-data projection Modeled revenue from nearby listings New purchase, no history Few true comps; optimistic blocked nights
Appraiser rent estimate Long-term market rent opinion Fallback or conservative path Lower number; ignores furnished premium

The most common projection tool is AirDNA’s Rentalizer. It estimates revenue, nightly price and occupancy from similar active listings within about 10 miles, matched on bedrooms, bathrooms and guest count.

The tool is a model, not a guarantee. One independent review notes that blocked calendars can read as demand and push projections higher, so the comp set deserves a hard look. Hospital-adjacent streets may have too few comps for the model to be reliable.

Where Does Stay Length Change the File?

Stay length changes the file because many programs treat stays of 30 days or longer as long-term rent. A furnished unit on 13-week travel contracts therefore usually lands in the long-term box, not the Airbnb box.

That matters because the furnished premium may not count. If a lender credits only long-term market rent, the extra income from furnishing and flexibility sits outside the ratio. It still helps the owner’s real returns. It just does not help the loan.

Mixed bookings create a gray zone. A property that rents nightly most of the year and occasionally takes a month-long guest is a different file from one booked entirely through agencies and corporate contracts. Contract-based income also produces no platform history, so there is nothing for the lender to read as short-term revenue.

Run it both ways before you commit. If the ratio only works on the short-term number, the file depends on the lender accepting the harder-to-document income.

What Does a Worked Example Look Like?

This example is a modeled illustration, not market data. It uses ratios only. The point is the sensitivity, not the numbers.

Scenario Income basis Modeled coverage
Base case Projected short-term revenue About 1.30x
Stressed case Same revenue, 15% lower occupancy About 1.10x
Long-term fallback Appraiser’s market rent only Around 0.95x

In the base case, the property clears 1.00 with room to spare. In the stressed case, it still clears, but the cushion is thin. In the fallback case, it lands just below 1.00.

That last result is not a dead end, but it is a decision point. Several paths may apply, and a lender would review each one:

  • A sub-1.00 structure, available through select lenders in the network, with leverage and terms adjusted.
  • A larger down payment, which lowers the payment and can lift the ratio.
  • An interest-only period or an extended term, available through select lenders, which can reshape the payment side.
  • A different property, if the numbers only work on optimistic assumptions.

Eligibility review depends on lender guidelines, credit approval and property review. If you are reaching for sub-1.00 because nothing in the area clears a standard review, re-examine the property before the loan type.

One more point matters here. A coverage ratio above 1.00 does not mean positive cash flow. Repairs, vacancy, management, utilities and replacement costs sit outside the calculation, and a furnished, high-turnover rental carries more of them than a standard lease. Stress the deal below the ratio the lender needs, not just above it.

What Should You Confirm Before Applying?

Confirm permission first, then the association, then insurance, then the lender’s short-term rental eligibility. Sequence matters because each step can end the deal before the next one is worth paying for.

1. Local rules. Short-term rental rules can vary by city, county, HOA, and property type, so investors should confirm local rules before relying on projected rental income. Parcels near medical campuses sometimes sit in institutional or overlay zones, so the check is parcel by parcel. One city’s published program, Raleigh’s short-term rental page, shows how permits and zoning districts can gate operation. Yours will differ.

2. HOA and deed restrictions. A property can be legal under city rules and still barred by covenants.

3. Insurance. Confirm the policy covers short-term use before you model the payment.

4. Lender eligibility. Not every program in the network takes short-term rentals, and the ones that do apply their own history and credit expectations.

5. Income documents. Gather platform statements or a projection report, the appraisal, and entity documents if you plan to hold title in an LLC, subject to lender program eligibility.

DSCR vs. conventional financing

There are two common ways to finance an investment property, and they qualify you differently — here’s how investors weigh them.

DSCR loan

Why investors choose it

  • Qualifies on the property’s rental income — no personal tax returns, W-2s, or pay stubs needed to document income.
  • No personal debt-to-income ceiling to clear, so existing mortgages and obligations don’t cap your borrowing the same way.
  • Can be closed in an LLC, keeping the property inside a business entity.
  • Built for scaling — not held to the limit on number of financed properties that conventional financing applies.
  • Underwriting centers on the deal: generally qualifies when the rent covers the payment, a 1.00x coverage ratio being a common baseline (confirmed in underwriting).
  • Designed specifically for investment property, including long-term and, where the program allows, short-term rentals.
Conventional loan

Where it’s strong

  • Often the lowest ongoing financing cost for a buyer who fully qualifies on personal income — a fit for a first property or a cost-first purchase.

Trade-offs for investors

  • Requires full personal income documentation and must fit within a debt-to-income limit — salary, existing debts, and other mortgages all count.
  • Typically held in your personal name rather than a business entity.
  • Caps how many financed properties you can carry, which can become a ceiling as a portfolio grows.
  • Evaluates you as a borrower as much as the property, which usually means more paperwork.

How investors usually choose: a first or single property often optimizes for the lowest financing cost; portfolio builders often optimize for leverage, vesting in an LLC, and scaling past conventional caps. The right answer depends on your goals, the property, and current guidelines — both paths run through select lenders in Lendmire’s wholesale network, with eligibility and terms confirmed in underwriting.

What Program Terms Should You Expect?

Short-term rental terms are tighter than standard long-term rental terms. The figures below are typical ranges from select programs in the wholesale network, subject to lender guidelines, and every file is underwritten individually.

Item Typical range
Purchase LTV Up to 75% on the strongest files
Refinance and cash-out LTV Around 70% on short-term rental collateral
Credit score About 640 or higher
Coverage floor 1.00 on purchases and 1.00 on refinances
Hosting experience About 12 months expected
Loan size Up to $3,000,000 on standard programs

Reserves vary by lender, leverage, loan size and transaction type, and are commonly around six months of PITIA. A bigger down payment can lower the payment and lift the ratio. It does not erase leverage caps, credit floors, reserve rules or property eligibility. The strongest files clear both tests: enough equity and enough rental coverage.

Manufactured homes, log homes and barndominiums are not offered through these DSCR programs.

Where Does the General Rule Break?

The general rule is that projected income can support a purchase. It breaks in a few specific places.

  • No history and thin comps. A projection built on a few distant listings is easy to challenge. A lender may fall back to the appraiser’s figure.
  • Conversions. Once hosting stops, underwriting reads the property as a long-term rental. The lender uses the lease, the market rent, or in many programs the lower of the two.
  • New construction. DSCR is a permanent take-out. It does not fund the build.
  • Demand concentration. One hospital’s staffing decisions, a hiring freeze or a strike can move occupancy. A file that looks fine at base case may fail the stress case.
  • Ratio versus reality. The lender’s number and the owner’s profit are different calculations.

Which Path Fits Which Situation?

The right path depends on the property, the history and the investor’s profile. This table summarizes the usual fits.

Your situation Likely path
New purchase, no hosting history Projection plus appraisal; expect scrutiny of comps
Existing nightly rental, 12 months of records Platform history as the income basis
Furnished unit on 30+ day contracts Long-term rent basis; model the fallback first
Existing rental converting to short-term Long-term basis until history builds
Local rules restrict nightly stays Mid-term strategy, underwritten as a long-term rental
Ratio clears only on optimistic income Rework leverage, restructure, or pass

A second-home loan is another option, but it limits how much the property can be rented. A DSCR file fits better when the whole point is rental income.

Common Mistakes to Avoid

  • Counting furnished mid-term premiums as if the lender will credit them.
  • Trusting a projection without checking the comp set.
  • Forgetting taxes, insurance and association dues in the payment side.
  • Modeling only the base case.
  • Assuming one hospital’s demand will hold.
  • Skipping the permission check until after the contract.

Key Terms Defined

DSCR (debt service coverage ratio): Monthly property income divided by the monthly housing payment.

PITIA: Principal, interest, taxes, insurance and association dues, the full payment in the ratio.

Short-term rental (STR): A rental on stays short enough that lenders and local rules treat it differently from a lease.

Mid-term rental: A furnished rental on stays of weeks or months, often read by lenders as long-term.

LTV (loan-to-value): The loan amount as a percentage of the property’s appraised value.

Seasoning: The length of time you have held or operated the property before a refinance.

Frequently Asked Questions

Can I get a DSCR loan on a hospital-area Airbnb with no booking history?

Often yes, on purchases, using a market-data projection or the appraiser’s rent estimate. Lenders typically expect about 12 months of hosting experience on short-term rental files, so history helps. Qualification is subject to lender guidelines, credit approval and property review.

Will a lender count travel-nurse income as Airbnb income?

Usually not if the stays run 30 days or longer. Many programs read those as long-term rentals and credit only market rent. Check the stay length before choosing the income basis.

Can I buy it in an LLC?

Many programs allow entity ownership, subject to lender program eligibility. Entity documents are added to the file.

What happens if hospital demand softens?

Occupancy falls and the coverage ratio drops with it. Model a stressed case before buying, and consider whether the property can convert to a longer-stay or long-term rental if needed.

Does a ratio above 1.00 mean the rental is profitable?

No. The ratio compares income to the housing payment only. Repairs, vacancy, management, utilities and turnover costs sit outside it.
